For New Cases
→ If you have not already started a case with us, please click the link below to fill out a service request and upload data at the end of the form.
For Existing Cases
→ Already filled out a Service Request? To upload new data for an existing case simply reach out to Case Management for a personalized data upload link. You do not need to fill out an additional service request.

Why We’ve Updated This Page
To help streamline our workflow and improve communication between surgeons, sales representatives, and our design team, we’ve updated the way case data is submitted. Previously, data could be uploaded without a corresponding service request, which sometimes led to delays and confusion.
By connecting all data uploads directly to a submitted case, we can ensure that each submission includes the essential information our team needs to begin work right away. This change will help us maintain accuracy, reduce back-and-forth communication, and return your completed cases more quickly.
